The Bridge Program is recognized by The Case in Point Platinum Awards for Disease Management/Population Health program
This May, the Bridge Program at Florida Hospital East Orlando was a national finalist in The Case In Point Platinum Awards, which recognizes successful case management programs across the health care spectrum. The Bridge Program is a free service for patients who are uninsured and suffer from chronic conditions. The program concentrates on care management during a patient's transition between the emergency department or hospital admission and home.
The Bridge Program received an Honorable Mention Award for the category of "Disease Management/Population Health". This category focuses on programs that administer to patients with chronic medical conditions in order to decrease the worsening of the disease and improve quality of life. The Bridge Program was selected for being an innovative and successful no-cost resource for uninsured patients in need of specific care management during hospital-to-home transition.
"We constantly strive to provide our patients with the best treatment possible. It's an honor for Florida Hospital to be recognized for our hard work and dedication to patient satisfaction," said Alba Santiago, program manager at Florida Hospital East Orlando.
The Bridge Program was in competition with other highly recognizable programs such as Catholic Health System and Genzyme. The conference was held on May 10, 2011, at the National Press Club in Washington, D.C.
